should get a flu shot? |

Anyone who...
• All children aged
6 months through 18 years of age.
• Has a chronic
medical condition including Diabetes, Heart Disease or
Respiratory Conditions such as Asthma.
• Is 50 years of age or older.
• Has contact with young children or seniors.
• Wants to reduce their chance of becoming ill
with the flu. |

• GCCHD will provide this service to children
during the regular immunization clinics starting October
13th. These clinics are held Tuesdays from 8 - 11
am and 12:30 - 3 pm at the Health Department located at
360 Wilson Dr., Xenia, OH.

• Children's vaccine will be available for all
children 6 months to 18 years of age.

Children under the age of 9 receiving the flu vaccine
for the first time will need two shots, one month apart.
Children who have received the vaccine in prior years,
whether one dose or two, will only need one shot this
year. |

• The charge for adults is $25.00 per flu shot.
|
| |
• The
charge for children $15.00 per flu shot.
|
| |

Medicaid managed care companies, including Care Source,
Molina, Amerigroup, or Medicare Part B accepted (must
bring card). Those with Medicare Part C and other
insurance companies will need to pay cash, then submit
receipt to their insurance companies.

Services will not be denied due to inability to pay. |
